The Connecticut Behavioral Health Partnership (CTBHP), in collaboration with the Connecticut Hospital Association (CHA) and Carelon Behavioral Health (Carelon), invites you to participate in a virtual statewide Community Care Team (CCT) Forum on Friday, May 31, 2024 via Zoom. 

This forum will address:

• Establishment and operationalization of CCTs in your community
• Key partners in the success of CCTs
• Managing CCT clients: eligibility, referrals, case management
• Communication between CCT members 
• Collaboration and shared responsibilities of CCT members

Participants will have the opportunity to learn from existing teams, ask questions, and discuss how to maximize the benefits of CCTs for the people served by CCTs.
