Statewide Community Care Team (CCT) Forum - onsite registration

The Connecticut Behavioral Health Partnership (CTBHP), in collaboration with the Connecticut Hospital Association (CHA) and Carelon Behavioral Health (Carelon), invite you to participate in a statewide Community Care Team (CCT) Forum on Thursday, November 2, 2023 at CHA in Wallingford or via Zoom. 

This forum will address:

·        history and the current state of the use of community care teams

·        related integrated care models in communities around the state

·        various approaches to operating CCTs

·        discussion on approaches to leveraging data collection among CCTs in CT to

o   facilitate the effectiveness and efficiency of a CCT

o   fortify advocacy to sustain such programs. 

Participants will have the opportunity to learn from existing teams, ask questions, and discuss how to maximize the benefits of CCTs for the people served by CCTs.
