Hospital Reimbursement

Connecticut's not-for-profit acute care hospitals serve as the state's healthcare safety net. However, with many hospitals' operating margins hovering at or below break-even for the past several years, and with demand for hospital services concurrently and steadily increasing, hospitals' ability to continue in this vital role is threatened.  

Because reimbursement is such a pivotal issue for hospitals and other healthcare institutions, many of CHA's efforts are focused on advocating for fair, adequate, and timely reimbursement to Connecticut hospitals for the services provided to all patients.

As part of its 2008 Legislative Agenda, CHA is working to:

  • Attain increases in Medicaid and State-Administered General Assistance inpatient and outpatient rates equal to the cost of providing such services.

  • Maintain funding for the Uncompensated Care Pool and Urban DSH Pool.
  • Expand critical access to capital funding through a CHEFA-issued revenue bond program to provide grants, forgivable loans, and low interest loans.

CHA Comments on OPPS Proposed Rule (September 14, 2007) CHA submitted comments to the Centers for Medicare and Medicaid Services (CMS) regarding the Medicare hospital outpatient prospective payment system (OPPS) proposed rule, which set forth numerous and significant operational and policy changes.

Faces of Medicaid in Connecticut   — A new CHA report conveys the implications of Medicaid underfunding to hospitals and the patients and communities they serve.

The State Legislative Program Review and Investigations (PRI) Committee released its report on December 18, 2006, on findings and recommendations regarding funding hospital care in Connecticut.  The full 69-page report, as well as a shorter " key points" document, are now available on the Committee's website.

Over the past 10 years, the combined operating margin of Connecticut’s acute care hospitals has averaged less than 1%. Click the following link for CHA's April 2005 Advocacy Update on Connecticut hospitals' 2004 consolidated financial results.

CHA commissioned the Lewin Group to produce a report on total Medicaid program expenditures in Connecticut, as well as a summary of hospital Medicaid payments and their impact on the fiscal health of Connecticut hospitals. CHA has sent the report, titled "Analysis of the Fairness and Equity of Connecticut Medicaid Hospital Reimbursement,"  to hospitals, legislators, and government officials.

SB 475, An Act Concerning Revisions To The Husky Plan, Part A And Part B

HB 5641, An Act Concerning Revisions To The Medicaid Program

CHA submitted testimony in support of these bills and encouraged the legislature to:

  • Establish a cost of living increase for hospital inpatient and outpatient Medicaid rates so that, at a minimum, the difference between funding and costs does not continue to deteriorate.
  • Return to hospitals the $27 million SAGA cut that was not needed to balance the budget.
  • Increase the ongoing annual hospital SAGA cap by $15 million so that SAGA funding can be on par with Medicaid funding.
  • Return to hospitals the $5 million Uncompensated Care Pool cut that was not needed to balance the budget.
  • Eliminate the next $5 million Uncompensated Care Pool cut scheduled for this July.
  • Restore declaration of income in the HUSKY Plan, Part A and Part B
  • Restore continuous eligibility in the HUSKY Plan, Part A and Part B
  • Eliminate cost sharing requirements (premiums and co-pays) in the HUSKY Plan, Part A
  • Fund outreach to help families apply for and remain on HUSKY
  • Ensure working families on HUSKY remain on the program for up to 24 months ("transitional medical assistance")

HB 5007, An Act Making Adjustments To The Budget For The Biennium Ending June 30, 2007

Patrick J. Monahan II, General Counsel and Vice President, Patient Care Regulation, testified on this bill, urging funding for recommendations advanced by the Lieutenant Governor's Mental Health Cabinet, increased funding to ensure adequate payment for mental health services, a budget provision that requires the Behavioral Health Partnership to provide for annual adjustments to all hospital rates at least equal to the average increase granted during the fiscal year by the Department of Public Social Services to HUSKY managed care organizations, and a commitment of resources that ensures proper placement of patients in need of mental health treatment and reduces the number of patients waiting in hospital emergency departments for mental health services.

CHA submitted testimony on these bills, which expand the HUSKY program by extending eligibility to uninsured persons with family income up to 300% of the federal poverty level and allow small businesses that currently do not offer health insurance to buy into the HUSKY program (SB 253 and SB 673), and provide for the establishment of a bipartisan commission to annually review reimbursement rates made to medical providers in any state medical assistance program and to allow such commission to increase provider rates to cover the reasonable costs of the services provided (HB 5688). CHA testified in support of this bill, which provides Medicaid increases for HIPAA implementation costs and for increased liability insurance costs. CHA requested that if the Committee acts favorably on the bill, it should amend the bill to include hospitals.
